Over-the-Muscle vs. Under-the-Muscle Placement: Pros and Cons

Subglandular (Over-the-Muscle)
For some women, placing the implant directly underneath the breast tissue can be the best choice depending on a patient’s unique needs and lifestyle. Potential benefits of a subglandular positioning include:
- Less implant distortion for bodybuilders and patients who perform pectoral (chest muscles) workouts
- Avoids stretching of the chest muscle, allowing a relatively faster healing period when compared to under-the-muscle placement
There may also be drawbacks to over-the-muscle placement, including the potential for less natural-looking results, as well as:
- May not be the best option for women with a thin or small body frame
- Increased likelihood of implant rippling when compared to placing implants underneath the muscle
Submuscular (Under-the-Muscle)
Placing implants behind the pectoral muscle can be a popular option for women who would like a smoother transition between the implant and the chest wall. The pros of submuscular implant placement include:
- Usually offers a more natural-looking appearance since there is greater coverage for the implant
- Less interference with mammograms and breast screenings
- Less chance of implant rippling
For some individuals, such as weightlifters and bodybuilders, it can be more advantageous to place implants over the muscle. There may be cons to under-the-muscle placement, including:
- May lead to a relatively longer recovery period than over-the-muscle placement
- Greater risk of animation deformity, which refers to squeezing of the implant or dynamic changes in the implant shape
